Dear Parents/Guardians,
On October 4, 2024 we will be having our annual Influenza (Flu) vaccine clinic at the school for all students grades PreK-12. The clinic is free of charge and provided by Millinocket Regional Hospital. The Influenza consent form and vaccine information statement were provided to all students to take home today.
In order to take part in this clinic, you will need to complete the consent form and return the form to school by Wednesday October 2nd. This allows me to provide Millinocket Regional Hospital with accurate numbers for the clinic. Only students who have returned their signed consent form will be able to participate in the clinic on Friday October 4, 2024.
If your child attends PreK on M-T-W and you are interested, please call to arrange a time to bring your child in on Friday October 4 for the clinic.
